Thank you for your interest in our breeding program and kennel and for considering one of our retired dogs/puppies as a new member of your family. We are selective about whom we sell one of our dogs and puppies to and this completed questionnaire will help us pick the best matching family for them. It is very important to us that we find the right home and family for all our kennel graduates where they will be loved and cherished. Completing this questionnaire will assist us in matching the right temperament and traits of a puppy with your family and your lifestyle as well as help us match the right puppy for any particular dog sports or task-specific training in mind. 8. How many members are in your family and their ages? Briefly describe your family. 

9. How active is your family, and what activities will your puppy be taking part in?

10. Living accommodations for you and your puppy. Where do you live? In what conditions will the dog live? Do you have a fenced yard? How will you exercise the dog?

11. Who will be the primary care attendant for the puppy?

12. How many hours a day do you expect the puppy to be alone?

13. Do you have a trainer or dog training school in mind? Describe the planned training strategy for the puppy.

14. If you're going on vacation, who will look after the puppy or dog?

15. Describe the grooming and care routine. 

16. What have you already set up for the new puppy in your household: crate, potty area, play area, safety equipment, and what safety precautions will be taken?

21. We require our pups to be spayed/neutered at the appropriate age. Do you agree to follow our contract (unless the puppy is being rehomed with a breeding/show rights agreement)?

22. If, for any reason, you are unable to keep your dog/puppy at any point during its lifetime, are you willing to return it to us to find a new suitable home?

23. Additionally, do you agree not to advertise your Last Samurai Kennel puppies as available for sale or free on social media, online, or in paper classified ads, but rather to notify us and work with us to return the dog to us?
